The system that Graham wrote is being hosted by CARET in Cambridge.

The steps for making this easy to use are only just being put in place. 
  Basically, to use the system you will need to set up an identity 
(using your email address) on the following system:

https://www2.caret.cam.ac.uk/selfserve/

When that is done, you'll need to email me and I can get your user name 
(your email address) added to the list of people permitted to upload to 
the following sites:

http://qtitools.caret.cam.ac.uk/qtiv2guest/guestformV2.html
http://qtitools.caret.cam.ac.uk/qtiv2guestpkge/servlet/GuestIntropkge?INIT

This is very experimental at this stage.  I'll be away for a week I'm 
afraid so I won't be able to process requests to use the sites until I 
get back.
