[QTI] n00b Questions

Unfortunately I don't know of any good source of samples other than the 
ones with the documentation. If you look at the various package examples 
and the files in the tests folder that should give you an idea about 
grouping items. You can also use constructr to create your own exampls 
(http://constructr.qtitools.org/), or use Steve's QTI converter to 
convert 1.2 files to 2.0 (download link on Pierre's site at  
http://www.gorissen.info/Pierre/qti/).

In QTI 2.*, unlike 1.*, there should be only one item in each XML file, 
and they should be put together in a content package.

I think you should probably start with the Implementation Guide 
(imsqti_implv2p1pd2.pdf) and then the XML Binding (imsqti_bindv2p1pd2.pdf).
