[QTI] n00b Questions

If there's no assessment node it is usually either an item bank or an 
individual item, although it could be a section of an assessment. There 
is an objectbank node available to explicitly say that it is an item bank.

QTI 2 was a complete rewrite, but also benefits from the experience of 
working with 1.2. The basic concepts are similar however QTI 2.* is 
closely tied to XHTML and removes almost all the ambiguities that caused 
problems in 1.2.1.
