Following on from Niall's comment . .
I am currently working on the import of QTI questions into WebCT, Blackboard and Moodle.
Moodle exports an almost working QTI v2.1, but does not import any QTI at all.
WebCT and Blackboard both export a sort of QTI v1.2, but do not import standard QTI v1.2.
They both ignore some of the QTI information and then use other information that you can
find buried in their exported xml, or they make assumptions about the layout of the xml.
Another route is to use Respondus - but that has other problems.
Another issue is that the VLEs support different subsets of features within some question types.

If you can live with the limited item types of 1.2.1 and are wish to
have your content delivered by major VLEs such as Moodle, WebCT etc.
then 1.2.1 is the way to go initially, but I think you should also aim
to be ready to update to 2.1 as soon as the major VLE vendors support it
because it is much better.
If you plan to develop your own assessment delivery system from scratch
I'd recommend going straight for version 2.1.
