[QTI] QTI 21.1 meta-data question

Hello All,

I have a set of test questions. For each question it
is accompanied by certain information, for instance:
a) Grade
b) Skill (which skill is being tested)
and so on.

I did read about QTI2.1 meta-data to see if I can use
this concept to write each question specific
meta-data. I did also look at the examples of ims QTI
2.1, which show the imsmanifest.xml. 
  
I am still not able to figure out how to put map the
above information in QTI 2.1 meta-data fields? Do I
*have* to put in the meta-data or there are fields in
assessmentItem itself to accommodate extra
information? 
 If no, can someone suggest me the mapping for above
information to QTI meta-data. Or maybe how an example
assessmentItem and its meta-data containing above
information look like?

Note: I was wondering if I can put some 3-4 fields
meta-data in the assessmentItem itself, so I do not
need to have an elaborate meta-data file?
