[QTI] More questions: AssessmentItemRef and True/False type

Hello,

While learning the assessmentTest format, I had the
following questions:
A) AssessmentItemRef
----------------------
1) I understand that testPart can contain any number
of assessmentSection and/or assessmentItemRef
2) assessmentItemRef points to an xml file containing
*one* and only one assessmentItem. The identifier for
which is given in the assessmentItemRef attribute.
Is this right?
  So if I have 100 questions/assessmentItems in a
section, would there be 100 xml files needed? 
  Or is there a way to put all assessmentItems in a
single xml file and let the assessmentItemRef point to
that file (with unique identifiers distinguishing
them). Please confirm.

B) True false in different form
---------------------------------
I wanted to know how to map the following question
into QTI. 
"This is a True/False question. If the answer is
false, the user should type in the right answer"

I have mapped True/False to multiple choice with
single cardinality. However was wondering how should I
write QTI format to let the user type in the right
answer and process the response?
